How we support people with complex care needs
We support people who may need extra help because of a learning disability, autism, Motor Neurone Disease (MND).
We understand that everyone is different. We take time to get to know each person and find out what matters most to them.
How We Can Help
We can support people by:
- Listening to their needs, wishes, and concerns.
- Helping them understand information about their health and care.
- Making reasonable adjustments so they can access services more easily.
- Supporting communication in ways that work best for them.
- Working closely with families, carers, and support networks.
- Helping people feel safe, respected, and included.
Supporting People with a Learning Disability
People with a learning disability may need information explained in a different way.
We can help by:
- Using clear and simple language.
- Providing Easy Read information where available.
- Giving extra time during appointments.
- Checking that information has been understood.
- Supporting people to make their own choices and decisions whenever possible.
Supporting Autistic People
Autistic people may experience the world differently and may have different communication or sensory needs.
We can help by:
- Listening to what makes appointments easier for each person.
- Making reasonable adjustments where possible.
- Reducing stress and anxiety during visits.
- Supporting communication preferences.
- Respecting routines, preferences, and individual needs.
Supporting People with Motor Neurone Disease (MND)
Motor Neurone Disease can affect movement, speech, and everyday activities.
We can help by:
- Working with other healthcare professionals involved in a person's care.
- Supporting communication needs.
- Helping people access the right services and support.
- Involving families and carers where appropriate.
- Focusing on the person's comfort, wellbeing, and quality of life.
Working Together
We believe that the person is the expert in their own life.
We work closely with patients, families, carers, hospital teams, community services, and partner organisations to make sure people receive the right support at the right time.

Meet the Team
The Team work as part of the wider all-age Safeguarding Team however in terms of complex care:
